Drinking water or chemical in a water bottle?

Today in a local daily, I read a person died two days after drinking sanitary fluid phenyl, assuming it as fruit juice. Earlier, I heard from a friend about his relative drinking hydro chloric acid (HCl) in a bottle assuming it as water and developing blisters in the mouth. Before that, I heard another incident of a boy taking water from a jug placed on a water cooler, that was used earlier for phenyl and had symptoms of stomach burning for few days.  There were many lab incidents due to the use of a chemical that is not same as the label on the bottle.
All above incidents stress on importance of proper labeling, safe procedure for handling of chemicals, proper hygienic practices and supervision. Otherwise, such incidents keep recurring at regular intervals.

To Prevent Accidents, We Must Change Attitudes

We develop attitudes over a period of time due to our past experience and as the experience repeats, beliefs are reaffirmed. Attitudes also develop due to influence from others. Wrong attitudes are due to wrong advice and changing these attitudes requires a lot of effort. It requires teaching with examples, being a role model, and may be some admonishing at the end. The article at the following link dwells upon changing the attitudes for accident prevention.

There are no exceptions. Follow the Rules, even if it is an emergency

In an incident, a coworker was assisting a crane operator in lifting the injured, by standing on a steel structure without a safety belt or any other protective gear. The employer was charged by OSHA for the violation.
This is an emergency case. But, many times, showing some reason or the other as emergency or important, people wilfully disobey the rules trading for short term gains. Even the experienced and senior colleagues preach such practices citing the urgent requirement. Funnily, the activities which were not attended for weeks will become suddenly urgent on the day they propose to take up and do not follow any safety procedure because they do not have time at that moment.
The example in the link below will serve as an eye opener to those who take chance by not following safety.

Safety incentives: Boon or bane?

Any incentive to employee towards safety programme implementation will help only in the beginning. Later on, they demand more and more which becomes a vicious circle and is like opening so called Pandora's box. If you try increase the standards after attaining and maintaining certain level, you will face resistance from the employees. In fact, as the article says, they may hide the issues and do not report, not only for getting the incentive, but that they may be rebuked by colleagues, if the person is involved/reported accident/near miss and deprived others of the incentives.

No increase in brain cancer from mobile phone use - Danish study

Earlier, we read from WHO study that mobile phone may cause brain cancer. Now, a Danish study says no increase in brain cancer due to mobile phone use. There are conflicting statements/reports from different study groups. Whatever it is, as electromagnetic energy can have biological impact, it is better to minimize exposure and follow the principle of ALARA i.e as low as reasonably possible.
If you are a business man and sitting at one place use fixed phone instead of mobile phone. If you are on the move, you can postpone your conversation till you reach your residence or office. If you are a marketing executive on the move, use earphones that are declared safe. Use of mobile phone on move also introduces other hazards like lack of concentration and can involve you in road accidents.
You may be lured by mobile applications, but the time spent on surfing or learning the applications is more than the actual gains and burning the pocket.

What is the safe voltage to work?

Many assume 24 V power supply is safe to handle. But an article in the link below shows that shock is possible even at 12 V, some sensation can be felt and secondary effects of such shock can endanger the person.
Even for DC supply, with a body resistance of 500 ohms in wet condition and 41 mA DC current leading to painful shock to women means 500 x 0.041=20.5 V.
People are reluctant to have earthing for the machinery saying that the voltage is less but we do not know what can happen and are ready to trade safety for a worst experience (shock!!!).

Electrocution during cleaning of welding machine

In an incident reported in a local news paper, a person died of electrocution while cleaning the welding machine in the morning. This implies damaged cables and improper/no earthing to the machine. Possibly the power supply was not switched off the previous day and in the same condition, the person would have carried out cleaning of the machine.
It is common to see use of structural columns as return lines and no earthing will be available neither for the machine nor the job on which welding will be done. Sometimes when proper length of cable is not available, people use rods or sheets or similar metal pieces as return lines. It is difficult to ensure safety in unorganized sector and even in organized sectors when the work is outsourced.

Fire accident in a ship at Kakinada port during welding

During welding in a ship, a fire incident took place leading to gutting of some rooms. During any welding, experts advise to maintain atleast 15 m distance around free from combustibles as sparks can travel this much distance. Otherwise, all combustibles in the area likely to be affected should be covered with fire resistant cloth and fire crew should be available in the area to avoid such incidents.

Cylinder explosion during unloading - 2 deaths, 1 injured

The incident of LPG cylinder explosion while transported with oxygen cylinders, root cause analysis, measures to be taken and a PDF file containing some photos are available at the link below.
http://www.citehr.com/360198-cylinder-explosion.html
(The content is reproduced as the author permitted sharing this information without restrictions when some other interested reader enquired in the forum in which this article appeared. Thanks to the author.)

INCIDENT :

Two fatalities & one serious injury occurred in one of the major steel plant, while unloading of gas cylinders from the truck as seen in the photographs.

Brief Description :

During unloading the Oxygen cylinders from a truck which is loaded with 95 Oxygen & 5 LPG cylinders, the oxygen cylinder hit the neck ring of LPG cylinder which gave way its welding causing LPG leak followed by explosion & fire.
Further the valve of the oxygen cylinder too got damaged & leaked causing explosion & the cylinder flown like a rocket. Two persons who were engaged for unloading were died & one person met with serious injuries.

Probable Causes :

1) The oxygen cylinders were stored in horizontal position & without protective caps.
2) LPG cylinders were kept along with oxygen cylinders.
3) Smoking in the area.

Lessons to be Learnt :

1) Gas cylinders need to be transported in vertical position
2) No gas cylinder is to be transported / handled without its protective cap. No cylinders should be received in our premises without having its cap.
3) Oxygen cylinder should not be transported along with cylinders containing flammable gas like LPG, Acetylene etc.
4) No smoking need to be strictly followed during transportation / handling the gas cylinders.
5) Gas cylinders need to be unloaded using standard method & not by throwing them.

Pl review the existing practices and takes appropriate actions. Please communicate to all, own / contractor employees about this incident.

Chlorine gas release - Over one hundred fall sick

As per the report (Toxic gas runs havoc) rupture of rusted pipeline carrying chlorine gas leads to atmospheric discharge and over one hundred persons from the factory, fire service and locals fall sick. The gas causes discoloring of leaves of the trees in the area.

The pipeline ruptured with a bang and workers inform that the pipeline is old and rusted. The fact that many of the sick were from the factory implies lack of communication, poor awareness about emergency actions and protective measures to be followed by the individuals.

Generally, vessels containing chemicals are tested but not the associated piping. Testing of piping along with containers should be made mandatory and the violators should be punished for endangering the safety of the living beings and the environment.

Lab safety - CSB advice

Safety is talked much in industries and followed to some extent. But, the research facilities and academic institutions, where experiments are carried out, this important activity is normally left to the wisdom of the individuals. No study is carried out about hazards in the experiments.

Chemical Safety Board of US recommends the following for improving lab safety.

1. Laboratory safety management plans must include physical as well as chemical exposure hazards.
2. Institutions should ensure that research-specific hazards are evaluated and mitigated.
3. Laboratory researchers need guidance documents to help manage hazardous chemicals that are unique to their research environments.
4. Research-specific written protocols and training are necessary to manage laboratory research risk.
5. Academic institutions must ensure that safety inspectors report directly to those with authority to implement improvements.
6. Incidents and near-misses should be documented, tracked, and communicated.

Chlorine leaks - impact on people's lives

Following is link that depicts shattering of many lives in US and this is true wherever chlorine and other chemicals are handled.

http://www.environmentalhealthnews.org/ehs/news/2011/chlorine-accidents

Instantaneous release of chlorine from normal tonners containing about 900 kg chlorine can impact living beings even at a distance of 10 km under worst conditions i.e overcast sky, no wind; with fatalities upto 1 km. Unless otherwise the safe handling procedures are not practiced, these chemicals are much danger than explosives that affect people and property only upto few meters whereas these chemical releases impact persons staying even hundreds of meters away.

How to Measure the ROI of Your Safety Program

Though, many times it is difficult to calculate the money spent on safety program, cost of accident, benefits accrued by implementation of safety programme, with extra efforts one can calculate and prove that the organization benefits from a safety programme that gets wholehearted support from all.

The link for indicators to be observed is given below. Meaningful quantification of these indicators can help in calculating cost benefits to the organization.

Hot water causes death of a boy

An young boy died from scalds due to fall of hot water bucket while playing nearby. The boy died two days after the incident while undergoing treatment. Many such incidents occurred as hot containers are within the easy access of innocent children who are not aware of the hazards. Neither the elders take care to keep such containers at height nor have watchful eye on the toddlers.
It may be noted that skin can withstand temperature only upto 42oC. Hot water at 70oC in 1 sec causes a complete loss of skin thickness in adults and 0.5 sec in kids (skin has cooling mechanism like blood circulation, sweating).

The remedial measures to be taken can be found in internet. One such link is listed below.

Honey bee attack - death of a boy

A boy died as his friends threw stones at a honey beehive and the disturbed bees attacked the boy nearby. It is common to see beehives outside many buildings and innocent persons become victims and many such incidents were reported in the press. As a student I heard an incident in which a person was attacked on the head by bees and there after the person used to cover the head with a cloth because of less hair. These beehives can be safely removed by getting expert help from pest control shops and is better to plan such activity when it is a weekend holiday.

Gas leak - six injured

An incident of gas leak and explosion was reported in the newspaper. Not knowing about the gas leak, in the early hours, the person lit a match stick to light a beedi that resulted an explosion with a loud sound and injured six in the house. An adjacent wall of the room collapsed in the incident.

As per another version in another newspaper, the house wife called her husband after smelling the leaking gas, who lit a match stick to see the gas leak.

Normally, there will be reports of cylinder explosion or stove explosion. But, in this report, it is clearly stated that the gas leaked, found ignition source, gas caught fire and exploded.

The incident is a lesson to learn about safe handling procedure for gas leak. After getting the gas smell, one should not switch ON or OFF the electrical appliances, open the windows and doors to allow escape of gas, take the cylinder to open area away from the residential buildings or cover the gas cylinder with a wet cloth, caution others about gas leak and avoid ignition sources and then call the gas cylinder supplier for necessary help.

Communication gap-Overhead electrical car rams trolley in a tunnel - 5 deaths

An overhead electrical (OHE) car that went for electrical track repairs rams a push-pull trolley with a supervisor and 4 gangmen who were on the daily check of the track. It is reported that it happened at a turning in the tunnel where gangmen might be unable to see the head lamp of the OHE car, unable to hear the sound because of electrically operated car and that there are no signals to use walkie-talkies being carried by both parties. Further there is communication gap between these parties and the adjacent station master. The OHE car was going towards Araku and is on downward gradient while the trolley was coming from Tyda. It is lack of understanding as two parties moving in opposite directions on the same track for the assigned jobs.

Obesity - Eating excessive food - Is it an addiction?

It is age old saying that, one should not talk while eating, chew the food 32 times before swallowing, take one glass water 30 minutes before and after taking the food but not during eating, take small meals for every 2-3 hours instead of 3 big meals in a day, take 8 glasses of water in a day and so on. But, for various reasons we do not follow most of such wisdom sayings.
As experts say, we take more food while watching TV, not knowing what and how much we are consuming. For the same reason, problematic kids also take food when they are watching TV. Parents switch ON to some cartoon channel to feed the children who otherwise will be resisting to have food.
I observed in my own case that I am grabbing some food item while watching TV, though I am not hungry. If I am not, may be psychologically I am feeling hungry and getting some item throughout the length of the TV programme. And, thus my weight increased 17 kg in the last 15 years, 4 kg of this in the last one or two months alone. More food is also consumed to avoid wastage.
Alarmed by this sudden increase, I decided to stop lunch for few days or may be few weeks till I get results on either way i.e good or bad. It's now 10 days. I observed that to stop hunger or burning in the day time, I am taking water and my water intake has gone up from one liter to about 3 liters during the day time. In a way, this practice is bringing my acidity problem under control, though my wife says it can be a temporary phenomenon. During this day time, I am taking one or two cup of tea or coffee, not more than that. Evenings, I am taking one or two handfuls of sprouts, which I started in the last one week followed by tea or energy drink (bournvita). Now, I am observing that my food intake during dinner also is reducing almost to half of the earlier quantity. I have to mention here that I am taking more side-dishes like sambar, rasam and curd with rice or roti. I feel like my stomach is full halfway through and unable to take any more food.
From the above, it's my conclusion that taking food is an addiction.
Now, I have to see for some more time, whether this type of eating food  helps in weight reduction also and develops into a food habit.

CSB video - W.Va. chemical leak

As per Chemical Safety Board (csb.gov.in) investigation, the three chemical leaks in 33 hr, causing death of a person are due to ineffective alarm, poor maintenance, failure to carryout proper investigation of near misses and emergency response system. CSB said that the person taking readings near the tanks, could have been saved if tanks had enclosures. The hose for phosgene transfer which leaked and sprayed on the person causing death, as per company policy, is to be changed once in a month, but was not done for the last 7 months. The report says, each of the three leaks were preceded by an incident (near miss- a warning sign). The suggestion of an employee for replacing with better material hose was not listened as it was costly. The incident made it more costly to the company. This proves again and again that safety is never costly and in the words of Mr Om P.Kharbanda, SAFETY IS FREE! - IN FACT IT PAYS!!

Close shave for 3 jets at Chennai airport - The Times of India

As per the reports (Close shave for 3 jets at Chennai airport - The Times of India), a major collision between a plane landing and another plane about to takeoff was averted due to prompt action by the the pilot of the landing plane just in time. The plane about to take off delayed its takeoff as a plane that landed before did not vacate the runway and was at the end of the runway. The taking off plane after aborting its takeoff abruptly by applying '(hot) brakes', was taken for inspection bay for checks.

It is stated that due to maintenance of second runway, total load of about 400 daily flight movements are carried on single runway.

Four workers in the first floor were charred to death as they could not escape from stairs that was filled with sacks and drums which were on fire. Fire tenders reached the spot without water. Police had difficulty to handle the situation in darkness without torch lights. This is a clear example of Murphy's law. Everything went wrong. The fire was extinguished after water was supplied by a private operator. Two officials were booked for negligence and causing injury.

Large reservoir of water in the universe

Scientists found huge water body in the universe around a black hole at 12 billion light-years distance. It contains 140 trillion times the water in the earth oceans. Is there any possibility of this water body coming closer to the earth or Sun any time in the future. Definitely a fiction movie can be made on such story. The web link containing the details is given below.

DGCA removes its top safety officer - Home - livemint.com

DGCA removes its top safety officer - Home - livemint.com
The above article is about removal of safety in-charge working for The Directorate General of Civil Aviation (DGCA) for delay in investigations and not producing reports in time.If the officer is inefficient, the action is warranted. But, in the same article it is mentioned that only 14 officers monitor over 1000 flights on Indian skies. With the increase in number of air craft, no. of incidents/accidents and other activities, the staff will be stressed to perform. And the issues are concerned with safety of invaluable human lives. Under stress, the performance will deteriorate over a period of time and the quality will dive to the bottom. After all, they are also human beings and not machines to generate output automatically after keying in certain parameters. If the problem i.e delay in reports, persists for long time, then the system needs to analyzed for problems. The issue of man-power and other issues like training need to be addressed before holding safety personnel as responsible. Airlines too should be held responsible for engaging persons with fake certificates, for incidents and accidents, instead of blaming one man. It may be a management principle taught to accept additional work, but will not help the organization in the long run when limits of human beings are exceeded.

A freak accident - unsafe act

It happened.

In a house, the family head tried to lit something. It did not. Then, he brought petrol in a bottle, poured a little on the material which he want to lit. After keeping the petrol bottle aside, he lit the matchstick. Immediately the fire spread from the material to the petrol bottle. In panic condition, he threw the bottle outside his house. The burning petrol bottle fell on a lady going on the road. She received burn injuries on hands and her clothing got burnt partly. She was admitted in a nearby hospital for treatment.
Mindless acts can lead to such type of accidents, endangering the persons who do not know what is going to happen to them.
Similar things can happen in industries endangering the environment and public at large due to unsafe actions by the persons inside the factory.

The fire occurred possibly due to a cigarette butt thrown on a pool of gasoline that got accumulated from a leaking pipe passing through densely populated slum area and where people gathered to collect the gasoline. As per one report, the pipeline might have been punctured deliberately to collect the fuel. Fire fighters had difficult time in controlling the fire. The leaking gasoline floated on a nearby sewer and it resembled like a burning river after the blast and resultant fire.
A report says, fire started after a fuel tankers dumped the fuel in the open sewer flowing through the slums.
If this is the reason, it is most unpardonable act and the guilty should be punished. If the reason is leak from the pipe line, then large quantity must have escaped from the line into the sewer till it is identified either by pressure drop or somebody noticed and informed the company. Both the company and the authority that cleared pipeline layout are responsible for the incident in this case. One can't lay the pipelines through populated areas. If the settlers came after piping job, then the company with the help of local police should have got it vacated. Such piping should terminate at a place far off from the habitation and should be protected. As per the reports, when a news channel asked the resident two years ago, he don't want to move from the place though he feels that there is a chance of the fire that occurred today.

Explosion at a nuclear waste treatment facility in France

A blast occurred at 1037 GMT today (Monday 12.09.2011) at Centraco nuclear waste treatment facility in Marcoule, France reportedly during melting of metal scrap. One person was killed, one was seriously burned and 3 others were slightly injured. As reports say, there is no leakage of radioactivity from the site.As per BBC reports, blast happened in a furnace used to burn waste, including fuels, tools and clothing which had been used in nuclear energy production but had only very low levels of radiation. The ingots formed after melting will be stored safely at a designated place. This is an accepted procedure.
Authorities informed that there are no nuclear power reactors at the site. The explosion also is not from waste storage tanks where radiolytic hydrogen formed can lead to explosion if not diluted. It is also not of any explosion that may arise during concentration of waste by evaporation.
Any non-nuclear incident in a nuclear facility can be misunderstood as nuclear explosion. This accident is to be treated as industrial explosion and not as nuclear explosion. If the explosion occurred during melting, one reason can be water reaction with molten metal. But, as per BBC report and other reports, explosion occurred after fire. It is not known the reason for fire which can be due to flammable solvents or due to friction of fine turnings of iron, zircaloy, aluminium, etc in the metal scrap. It is a known fact that dust explosions are possible with fine dust of these metals.
As there is no radioactivity, we have to assume the explosion is non-nuclear of one of the above types.

Power failure due to snake in a feeder box

A snake entered a feeder box from which power is supplied to an industrial area. The snake died of electric shock and at the same time, power supply to the industrial area also got cut off. Concerned officers of electricity board could not find the reason for power failure for more than 10 hours. After search, they found 1.5 m long dead snake in the feeder box. They claim that they did not see such a problem earlier. Production loss for 10 hours, environment pollution due to DG (diesel generator) sets operated during the period, revenue loss for the power supplier and the inconvenience caused to public will add to large amount, though the cost of preventing such incident i.e ensuring no openings, is minuscule. Many times, we see open fuse boxes, partly opened doors of power distribution boards, etc which can lead to entry of reptiles and causing such damages and this has become an accepted norm for such industries.

About Leadership - excerpts from the book by Robin Sharma

Excerpts from, “The Leader Who had NO TITLE” by Robin Sharma, which I liked the most. I read this book sometime back and made the notes which I reproduced below. After reading, I felt that by following the tips, we can improve our personal life and have peace of mind with satisfaction. This is true for improving safety also at work place which will give satisfaction of saving lives, avoiding property damage and protecting the environment.

The website link is here: http://www.robinsharma.com/

  1. Learning is the daughter of repetition. Repetition is a powerful teaching tactic. Through repetition, a new idea can quickly become integrated as a new belief.
  2. Success is easy for anyone, if they do right things consistently. And these little things and behavior buildup over time lead to reaching the impossible, which appeared in the beginning. Seeds do not give fruits overnight. They have to be planted, watered, weeded and fertilized before we get fruits.
  3. Small insignificant daily acts of excellence buildup success. Neglect of small daily acts over a period of time lead to no point of return and failure.
  4. Success is achieved only when we practice and experience. Listening or speaking alone will not help. When we practice the idea that we listened, it becomes a truth and we experience it. Unless the idea gets into us and we practice, listening/reading fails to crate lasting results.
  5. There is nothing impossible in this world. Only it is a matter of time, after we put our sincere efforts.

Why people do not want to learn from incidents?

Everyday goes in our life without some incident that would have caused rise in heart beat. If it is during the driving on the road, either we shout or get earful hearing from the other side. Here, always the bigger vehicle driver is at the receiving end. If it is at some other place, like at home, work place, bus, train, or market place, both sides will be vocal in airing the views.
Every time after the incident, consciously or unconsciously we try to analyze what happened and who is responsible. Most of the times, we conclude that we are not wrong and that the incident happened because of the other person. After ascertaining this again and again in our thoughts, we feel satisfied that we did nothing wrong. If we were the affected during the incident but were at the receiving end, we think that we can’t do under the circumstances as we are less in numbers. If, someone got injured in which we are also a party, we tell ourselves that we are right and feel sympathy or that they deserve it for not being correct.

Artificial resuscitation - video link

For adults, during cardiac arrest, this video shows continuous chest compression can lead to supply of oxygen rich blood to the brain and increase survival chances. One may think that by this method, the patient's chest bones may be broken. But, when he is already in danger, bringing life back is important.

Faulty transformer - shock to the village

Two persons died, 10 persons got injured and several electrical appliances got damaged when an entire village was under electric shock. The two persons died when they tried to switch on power supply in their homes. The news item appeared in Times of India. The report says this happened due to a faulty transformer and after the identifying the problem, the transformer was replaced and power supply was restored.
I talked to my friends of electrical engineering discipline on this news item. I was told that this can happen, due to lack of earthing, or supply of high voltage directly to domestic users, or when there is contact of phase with the body of the equipment with no neutral. If persons are wet, then they can get shock while switching the plug.
Earlier also there were many news reports of damage of TVs, refrigerators, bursting of picture tubes, etc due to high voltage.
