Fatal oil tanker fire - greed of persons to collect oil

Fire from overturned oil tanker causes more than 150 deaths. Upon hearing the news of fallen tanker, nearby residents rushed to collect and died from the flames. They should have thought of fire hazard. No amount of training can help in such cases. Greediness is the culprit.

Liquor bottle in trouser pocket breaks - person dies

A person kept liquor bottle in his trouser pocket. He ran when it rained and fell. Liquor bottle broke and its pieces injured private parts. Person died while undergoing treatment.

Teacher gets burns during demo in the chemistry class

In a fire incident, a teacher got first and second degree burns while conducting an experiment in front of the students in a chemistry class. The school science classrooms had emergency showers and the teacher doused flames by using it.
It is highly appreciable that the school management has kept emergency showers in the classroom which is rare.
However, one could have checked the material safety data sheet (MSDS) for the chemicals handled before the experiment. Or, was there any slip during the experiment, though the hazards are known?

Hazards in the job of trainers?

Recently, I delivered a talk in a safety training programme. After the lecture, I went out of the training hall, and tried to talk to my office for vehicle pickup to my workplace. One of the participants came to me and advised me to remove portable microphone, which was given to me before the talk. My phone conversation was audible in the training hall.

This is a lesson for me. I should have switched off the mic and handed over to the program coordinator. I forgot this. The other side is whatever I utter also will be audible and any loose talk (which may happen) would have put me in shame, if not in trouble.

In the video, a woman carrying hot water slips on a smooth surface having some spillage and the hot water splash on her body causes scalds.
The video in the above link is old and seen earlier also. However, it is important to note that many do not care about smooth slippery surfaces that are because of surface finish, spillage of liquids, etc. Use of improper shoe, or presence of congested areas, etc add to the hazard. Many even question about considering such surfaces as hazard.

Pray God that nobody should undergo such punishment.

Safety work permit - violation of conditions

 Safety work permits are issued after ensuring that the site conditions are suitable for safe work. Check points are verified and are certified for start of work. However, still sometimes accidents take place indicating that either job hazard analysis is not done properly, conditions are altered during the course of work, workmen are not briefed about hazards and measures to be taken, many times they are not aware of what is written in the permit and simply do the work as they were told or as they can do, lack of supervision, etc.
In a huge manufacturing facility, a number of works will be going on and generally, the supervisor will be monitoring more than one work at any given time. In such a condition, at least, emphasis should be given to training to the persons engaged. All are not of superman category.
But, when earnings are motive, all other aspects take back seat. Many times, procedure for selection of the contractor or agency to do the outsourced work is the culprit. If selection is made without assessing the competency of the person/agency for safe execution of work, without checking the credentials, without giving proper description of the work, then we will get a wrong person in most of the cases.
In some cases, trainees are placed directly on to the job without proper class room training and supervision. This also leads to accidents as they are in the organization to learn, initially in the class room and then to have on the job training under supervision. Sometimes, these trainees even will be manning the shifts, particularly at night times when availability of regular work force is less and may be when nobody watches. Though, this is disastrous, shop floor managers take such decisions without taking the concurrence of higher ups. If seniors also give consent, then it is silly and such persons should be terminated or they should be asked to supervise instead of posting trainees for manning the shifts / works.

Know about eye wash shower

An article in detail about eye wash shower location and care to be taken is available in the following link.
I learnt from this that the eye wash shower should be reachable in 10 seconds, to be located at same level of hazard area and dust covers should be provided to avoid contamination.
It is also stated that ANSI requires 30-90 psi pressure. It is my feeling that high pressure water should not be used as in the literature, hazards of high pressure water jets on eyes state damage to the eyes.
As per Indian Standard, the pressure to be maintained is 196 kpa i.e about 30 psi and 16 lpm flow rate for eye wash fountain and for emergency shower, it should be 110 lpm. As per this standard, it should be located within 15 m distance from chemical handling area.
The reason for minimum 15 minute cleaning is that even after this much cleaning, only about 95% chemical is washed off, which I read in internet written by an author from US about HF splash on the lab coat. I used to explain this to my audience in the training programmes about the necessity for washing atleast 15 minutes. Otherwise, human tendency is to wash till such irritation exists. However, the vapors from the chemical soaked cloth can continue to expose the persons and cause health effects later. And, HF will show delayed effects affecting lungs and exposed persons can die due to odema.

The report of safety audit points to temporary nature of jobs that have become permanent, like cabins, partitions, loose wiring, addition of air conditioners, piling of broken furniture along the stairs, etc restricting/altering access, creating hazards and putting the lives of the persons in the building at risk. It is reported that the furniture is removed based on the audit report and canteen kitchen is being moved to avoid use of gas cylinders as explosives and recommended for use of piped gas. Though it is not reported, vehicle parking also should be looked into from safety and security aspects. It will be a Herculean task for the security officials to screen these vehicles apart from the persons, but requires attention.

Location of transformers closer to buildings, walkways - a hazard

In an article, it is reported that many transformers located adjacent to foot paths or closer to buildings though as per Petroleum Act, atleast 9 m gap should be maintained. As per Indian Electricity Act, at least 2.5 m vertical and 1.5 m horizontal clearance should be maintained for 11 kv lines which is hardly followed. Read the full information in the following link.

Hot water causes death of a boy

An young boy died from scalds due to fall of hot water bucket while playing nearby. The boy died two days after the incident while undergoing treatment. Many such incidents occurred as hot containers are within the easy access of innocent children who are not aware of the hazards. Neither the elders take care to keep such containers at height nor have watchful eye on the toddlers.
It may be noted that skin can withstand temperature only upto 42oC. Hot water at 70oC in 1 sec causes a complete loss of skin thickness in adults and 0.5 sec in kids (skin has cooling mechanism like blood circulation, sweating).
