Importance of readily available data for preventing disasters - an article

Recently, I came across an article from Kinsmen Group, as given below. It discussed the importance of readily available data on workplace activities and incidents to prevent workplace disasters and protect the employees. It is called smart data, in the news item. 

Following are some of the issues and observations made (data seems to be for USA):
  1. As per OSHA and Labour department, in the year 2019, more than 5000 persons died 9,00,000 were injured at workplace.
  2. Companies spend an average USD 7,500 for each lost-time workplace injury (we can call this as direct cost). Other costs consist of replacement of damaged equipment, reconstruction of facilities, compensation, etc. (As per theory, direct cost is only like a tip of the iceberg. And, cost of preventing an accident will be only about one fourth of the direct cost).
  3. As per National Safety Council, cost of workplace injuries in the year 2018 was USD 170 billion.
  4. Each fatality costs USD 1.0 million in medical costs, employer costs, and wage losses
  5. In a fatal fire and explosion incident, it seems, the emergency personnel do not have information of chemicals involved, otherwise, they could have saved the lives. 
  6. Digitization and availability of data easily is not yet compulsory by many certification agencies. Organizations follow data retrieval in general age old methods like physical search instead of going for easy methods by using the computer/cloud technology/internet
  7. Data should be latest
Many of us definitely would have spent a lot of time in search of some information. It is my personal experience that on most of the occasions, I spent more than two hours to retrieve, and sometimes more than one person was engaged for information retrieval. And, on some occasions, I didn't find what I want. Availability of signed information will help us to support what remember/believe and use it to convince others for implementation of safety or whatever we would like to.

Digitizing the records with a provision to search will be helpful for us and our future generation. Some believe digitization means scanning the documents. But scanning without search option makes it useless. It is a primitive form. Instead of paper search, we have to search in computer page by page. Indexing these types of scanned pages may make life somewhat better. But, if scanning of documents is in readable text form to enable search with key words, and is available in encrypted form in cloud, like dropbox, mega, onedrive, pcloud, etc and is accessible with password protection to prevent unauthorized access, it will help a lot for productive use. 

Searchable scanned records helped us greatly in the analysis of similar incidents and find suitable solutions to prevent recurrence of the incidents for a safe workplace  

Updating of data is a continuous process. 

Knowledge management is very important and critical to ensure safe productivity. World renowned safety professional Dr Trevor Kletz stated in one of his books (What Went Wrong) that incidents keep repeating either in the same or different similar organizations. We have to learn from them and implement measures to prevent recurrence.

Another safety professional Mr O.P.Kharbanda stated in a booklet about Bhopal accident, that Safety is not costly. In fact, it is free. What we need is the will to implement. 

In one of the investigation reports, EPA made an observation which states, it is not the lack of knowledge that is responsible for the accident. It is the lack of use of the available information that is responsible for the accident.

How we can get the information at first place. If it is made available in a readily searchable form and is known the personnel how to use it, then this information management will help to achieve what we intended to.

Qualified technologies and personnel should be assigned for this and definitely organizations will reap the benefits from this.

Poor safety management - cause for blasts in chemical factory leading to 23 deaths

The chemical factory stores vinyl chloride (chloroethene, chloroethylene) in three tanks. Two tanks were full with the chemical at the time of the incident. Due to leak, pressure in the tank dropped. Instead of finding the cause for pressure drop, operators pumped more chemical into the tank. This lead to bursting of tank and release of chemical. Ignition source from outside the plant caused explosion of the leaked chemical, burning the trucks and cars outside the plant. 23 persons died and 22 were injured in the incident.

Fire and explosion in pharma factory


In an incident of explosions in pharmaceutical factories that stored combustible chemicals, one person died, few persons suffered burn injuries, 3 of them serious. As per the reports, it is not clear whether it is boiler explosion or chemical reactor explosion.  Blast could be felt 12 km from the factory site. Window panes of buildings at 3 km were shattered.

Chemical reactor explosion kills one, injures three

As per news report in The New Indian Express, an explosion in a chemical reactor in a bulk drug company, caused death to a person and injuries to three persons.
The metal piece of reactor after explosion, flew by 100 meters, passed through by breaking a wall and hit the person working at distance. The person died immediately due to head injury.
The incident reveals dangers of flying fragments from explosion and need for assessment of explosion consequences.

Blast/explosion during welding on oil tanker dismantling causes deaths/injuries

Loud blast due to explosion during welding on a oil tanker parked in ship breaking yard, caused more than 10 deaths and injuries to about 50 persons. 8 explosions took place during the incident. A fire followed the explosion. Rescue efforts were hindered due to explosions and fire.
It is stated that about 100 persons were working in the ship tanker at the time of the incident.
To protect themselves, some of the workers jumped in to the sea, after the explosion.

The photos of the incident reveal massive destruction can be seen.


Earlier, three months ago, in another blast during explosion on a oil tanker in oil terminal, during welding, three persons died and two were injured.

LPG explosion during welding inside a pipeline



While doing welding inside a pipeline below the ground level, with LPG gas cylinder outside at ground level, there was an explosion inside the pipeline. Unable to escape, four persons inside suffered burns. One person received 87% burns.

The reasons for gas leakage into the pipeline are not known. Poor quality of hoses, not following confined work procedures (like checking the flammable atmosphere, oxygen levels, having arrangement for emergency escape, buddy system), etc can be possible reasons.

Explosion in petrol pump - tank thrown 500 m away


In an explosion in petrol pump, the tank was thrown 500 m away, captured by a passerby in mobile.

One person died and 17 injured in the incident. Flying of the tank can be seen in the video linked above.

Watching fires is not a fun

In the video, it is seen, persons walking aside, not knowing the dangers of such fires. Fragments from explosion can travel great distances and can kill the persons. Thermal radiation from fireball also will be felt at longer distances, the overpressure in the form of pressure waves can lift the equipment/persons off the ground, making them to hit other objects and thus get injured/killed.

Locking of emergency door leads to death in a fire incident

An explosion and fire during filling of gas cylinders led to the death of a person (apprentice) doing the work. The main door was blocked by flames. The second door was locked and could not be opened in time resulting in suffocation and burn injuries related death.

Few incidents occurred earlier also in PEMEX operations. The famous and widely referred in text/reference books and websites including Loss Prevention by Frank P Lees is the one that happened in 1984 where about 500 persons died. This 1984 accident happened just before another accident in Bhopal.

Explosion in detonator factory during disposing of rejected detonators from explosives

In an explosion in one of the largest explosives exporting factory, during separation of rejected detonators from explosives by cutting wires, a mechanical cutter fell down on the explosives below causing the blast leading loss of two lives and injuring 13 others with some of them receiving severe burns.
One of the injured suffered hearing loss during the incident.
The site was visited by State Home Minister, Police and other officials.
Police registered case under various sections of IPC (Indian Penal Code) for negligent act and others leading to personal harm and initiated the probe.
As per reports, the persons doing the job were retired employees as contract workers having experience.
There were also reports of demanding for shifting of the factory from residential premises.
There were some fatal incidents earlier also in the last 10-15 years.

To avoid such incidents, possibility of  automation of removal of rejected detonators needs to be explored.
As the cutter fell down below on explosives during the job, and it is likely that such slippages can happen, suitable path for shifting of material below as soon detonator is separated from the explosive material for every explosive or collecting the explosive material under water or any other medium has to be seen.
Governments have to ensure that industrial areas are well separated from residential areas. There should a sort of no habitat zone between industries and residential areas. It is natural that wherever development is there, the surroundings will see growth of residential areas with a view to gain from real estate price increase. 
Definitely, large industrial complexes would have been set up far away from residential areas at that time. Earlier remote places would have become part of the present cities due to growth. Shifting of industries also will be costly and who will foot this cost. 
Hence, a policy as suggested above is required with marked zones and no-man zones.

While doing gas cutting on a car was not in use for more than 20 years, for salvaging some parts, presuming that the car tank would have been empty, led to vapor explosion. Property worth 25000 USD is damaged and there were no injuries.



The incident tells us that we should not take anything for granted and we should ensure before start of work that there are no hazards.

Ammonia reactor blast - 10 persons injured

As per news reports, a brine unit chiller with ammonia exploded on Saturday i.e 28.06.2014, leading to ammonia leak, injuring 10 persons with burns. As per some other reports, 11 persons got injured in the incident. Persons suffered suffocation, breathlessness and giddiness.
To clear the choking of brine in the chilling unit, steam was admitted which led to expansion of ammonia (anhydrous?) and pressure rise that causing explosion.
As per reports in some other news papers, use of steam instead of hot water led pressure rise. Hot water use limits the temperature compared to steam and may be this is one of the reasons for the incident.
Reports indicating fire extinguished rise questions of whether it is ammonia fire or any other fire.

Blast during dismantling of a chemical boiler - 3 persons die due to burns

Three persons died in a blast that occurred during the dismantling of a chemical boiler in a scrap godown. The contents of the boiler are not known. However, this incident is a lesson that any vessel before sold as a scrap should be cleaned thoroughly to ensure that it is free from chemicals. The incident might have happened during gas cutting of the vessel with trapped chemicals generating hot gases or undergoing runaway reaction and with improper venting. Provision for venting should be available to avoid pressure rise.

Cockroach chase leads to factory fire - how to eliminate cockroaches in your place?

Today, I read in a forum, i.e indiansafetyprofessionals@yahoogroups.com, that a house wife died of burns when she sprayed a cockroach killer in the kitchen near the gas stove which was ON. The spray, being flammable caused explosion.
Husband of the lady who tried to extinguish flames is still in the hospital due to burns.

Then, when I searched internet for similar incidents, following links were found.

Best solution I found to get rid off the cockroaches is to use the latest HIT GEL and I am not finding any cockroach in my flat for the last few months (more than three months). Occasionally, when I find something, I use it and my house is free. Otherwise, there were hundreds of cockroaches, hidden behind tiles / corners / books / papers, etc and being in apartment complex, I thought no amount of treatment will solve my problem as they can come from other flats, even if I eliminate in my house.
