Preventable accidents, if they are not prevented due to our negligence, it is nothing short of a murder - Dr. Sarvepalli Radha Krishna, 2nd President of India

Zero accidents through zero unsafe behaviors. Do not be complacent that there are no accidents. There may be near miss accidents (NMAs). With luck/chance, somebody escaped knowingly or unknown to the person. But, we can't be safe, if we depend upon the luck.

Safety culture is how the organization behaves when no one is watching.

Slab fall from demolition of adjacent flats damages common boundary wall

As per news reports, during the demolition of flats, a slab falls down on adjacent common boundary wall, causing damage. The area on this side was  not barricaded, though, it was barricaded on other two sides by the construction company. The debris and dust during the demolition will be an issue for the neighbours and needs cordoning for sufficient distance as a protective measure.

As per statements, in the same report, there are no rules governing demolition buildings.
It is fortunate, that nobody is injured in the incident.

Who is responsible for safety for works in your plant?

In an operating plant, various works of maintenance, civil, erection/commissioning of new equipment/facilities, house keeping, etc will be in progress under the supervision of in-charges for respective agencies. However, in case of any lapse with regards to safety or any incident/accident, question arises to decide who is responsible for the incident. Though, safety permits may be issued in the name of the executing agency, plant in-charge also should be held equally responsible as it is the work in his plant and is being done by other agencies because of his request. If any incident occurs, brunt is to be borne by plant officials, plant operations may have to be suspended and in worst case plant may be exposed to damage.
Hence, plant officials can't escape from responsibility to ensure safety during works executed by other agencies.

Sharjah blast damages over 10 shops, 15 cars

Sharjah blast damages over 10 shops, 15 cars

There is a blast in a cafeteria due to gas leak from LPG gas cylinder that caused damage to 10 shops and 15 cars. We underestimate the destructive power of domestic LPG cylinders and nothing happened till now does not mean that we are safe. We have to check the condition of the cylinder, condition of the regulator and condition of the hose. We have to turn off the regulator when not in use. It is common only to close the knob on the burner instead of closing the regulator also. There were many incidents of passersby on the road getting injured/died because of fragments generated during the blast hitting them.
