Zero accidents through zero unsafe behaviors. Do not be complacent that there are no accidents. There may be near miss accidents (NMAs). With luck/chance, somebody escaped knowingly or unknown to the person. But, we can't be safe, if we depend upon the luck.

Accident prevention - enforcement by mandatory publishing of safety data on product labels
Further, it is stated that the reputation is also one of the factors for ensuring safety in the organization.
The reasons listed above, for investment in OHS are correct. However, I am not sure how many will see the reputation of the company with regards to OHS aspects, while buying its products.
Job aspirants and organizations engaging the services of organizations may take a step back and think before joining or using the services as they do not want to harm themselves or see delay of their projects. But, for this OHS data should be available before taking the decision.
I am doubtful about reputation aspect while buying a product as it does not make economic sense for anybody to pay more to products of another company because of poor reputation (on OHS management).
To drive home the message about OHS issues, may be, it should be made mandatory to display the safety statistics (readable font size and color) on their product labels. For example, 100 g of xxx biscuit packet contains 2 g calcium, 1 g iron, .......; and 2 persons injured in accidents during the making of this biscuit batch during the period ----. It is like blood stain on the product, made to be visible in the form of data display.
A rating system also may be thought off by governments or independent rating agencies that can be displayed and seen easily to enable quick decision by conscious public.
Nobody likes to display such information and thus may be more interested to ensure health and safety. And, once it is mandatory to display safety data or rating on the product labels, definitely, it will make manufacturers to be more sensitive (atleast to sell their products) to OHS issues.

Some thoughts on culture / safety culture
If everybody works safely without monitoring / reminding / without the need for display of posters, slogans, etc then really safety culture has been imbibed in their blood!!!.
It means, there is no need for safety department and can be disbanded as every employee has taken that role on himself.
A leader should not get blinded by reports from few lieutenants / spies (the spoilers who increase the distance between the leader and subordinates) in the organization to know what is happening. Everybody should be able to communicate to him so that he gets a fair idea of what is happening and how things are moving in the organization. Once such reporting is established, it aids in safety culture also.

Fines should be such that violations are not taken casually. Punishments are better than fines. This is applicable in all countries. Violators should not find benefit / profit in violation.
Everybody says safety first. But, do not explain the reasons for injuries/deaths and where they erred, unintentionally.

Success or failure - it is a team's achievement - with a rider!
If the team members are hand picked without choice or concurrence of the team leader (Captain), then the committee is responsible for the outcome. The role of the captain is limited to running the show, as he can. If the captain is not capable, then a good team also can not perform.
This is also true for OHS management. You appoint some staff who are worthless and expect the result, it is only a daydream. For name sake, there will be X number of staff, but real performers may be scarce among the X.

Ammonia reactor blast - 10 persons injured
To clear the choking of brine in the chilling unit, steam was admitted which led to expansion of ammonia (anhydrous?) and pressure rise that causing explosion.
As per reports in some other news papers, use of steam instead of hot water led pressure rise. Hot water use limits the temperature compared to steam and may be this is one of the reasons for the incident.
Reports indicating fire extinguished rise questions of whether it is ammonia fire or any other fire.

Bullying safety officer is like hitting under your own pants
Clearly, there are provisions in related regulations to penalize and also punish the works managers for violations. It is a matter of time before the law catches up with the violators.
Unless these works managers realize the importance of safety and learn the punishments that await the (willful) violators, there is no light for the safety of the persons working in work areas managed by such persons who are no different from murderers.
Injuries / deaths to employees in those work areas can be compared to planned murders/attempts to cause injuries as works managers neither take action to prevent unsafe conditions not help the safety professionals to identify and suggest safety measures.
Unfortunately, safety officers are not vested with powers like that of regulators and thus will be at the receiving end unless the in-charge of the facility gives unconditional support. Else, at some time or other, the safety department officials either will keep quiet leaving the matters to the fate or will look for better opportunities in other companies. Either way, it is the loss to the company having such safety culture.

Crowding near the entry point of examination centers
Inspite of knowing this, the accomplice of the candidates like parents, friends, relatives, etc were found standing near the gate of the test center even after their wards went into the examination hall, thereby obstructing the movement of the candidates reaching the center from different places of the city and state.
This will be intriguing to those who rush at the last moment delayed for various reasons, self inflicted, traffic, not knowing the location, etc and because of no entry even for one minute late, they will be at loss and have to lose one academic year.
Nobody should be allowed to be present near examination centers and its entrances to avoid such situation.
