Zero accidents through zero unsafe behaviors. Do not be complacent that there are no accidents. There may be near miss accidents (NMAs). With luck/chance, somebody escaped knowingly or unknown to the person. But, we can't be safe, if we depend upon the luck.

Scaffold erection - work at height - construction safety

I booked a flat and work is in  progress (for last three years!!!). Today, I attended a meeting of flat owners with the builder. Like earlier times, again date for handover was extended by a month. The reason told was that because of rains and high velocity winds at height, and as it involves erection of scaffolds i.e work at height, construction activity was stopped.

Whatever was in other owners' minds, as a safety professional, I was glad to hear such a thing that builder stopped his work inspite of project delay. Already, he is running beyond the agreed possession date which was almost two years ago and is liable to pay some compensation as per the agreement.

How many will take such a decision though it is well known fact that any untoward incident will lead to project delays due to enquiries by police and municipal authorities.

Construction is an activity in which large number of accidents take place and the work force is also mobile with little protection for their lives.

Fall from height - wrong use of forklift - firm penalized

In an incident, two staff of a door fitting firm strapped a wooden crate to a forklift to go up and fix a motor on the wall for a rolling shutter. The person was lifted on the attachment made to forklift and when he turned to pick tools, the crate tipped, person fell by 5 m height after losing balance and received fracture injuries. He was operated upon. HSE investigated the incident, prosecuted the employer and imposed a fine of 1000 pounds and 1870 pounds for costs because of poor planning for work at height and violation of work at height regulations.
In many work places, it is common to see abuse of forklifts. They are used for double/triple/ quadruple riding, joy riding, to go to other departments on personal work, use forks for reaching an elevation instead of using ladder, etc. Employees also do not listen to safety supervisor's objection and it is distressing to watch that even in-charges also take it as normal and no other go option.
In a regional movie which I saw several years ago, the hero who is MD of a soft drink company (brand displayed is reputed) and his friend both stand on the forks with hands resting on the body and moving around. I do not understand how the company allowed such act in their premises, while permitting the film unit for shooting.
Fatalities, injuries like amputation due to trapping between moving fork and stationary mast, hit against, loss of control due to driving at speed, toppling while driving on inclined roads or at high speed or loose ground, etc were reported and one can find such case studies in internet. Still, people take it as one-off incident or of the attitude that it will not happen to them.
Safety at work place can be improved only when fellow employees question unsafe practices and force the violators to stop such acts.Good words and rules will have impact on those who listen but not on wishful violators.

Heavy winds upturn 7 containers and flyoff 2 drivers

There were reports of heavy winds in Srikakulam district, Andhra Pradesh, India on 10.06.2011. Seven containers on the road were upturned, two drivers on the road flew 20 feet high and fell down receiving injuries and broken bones/joints, many electric poles, houses and trees were uprooted. The wind speed was about 500 kmph. Incidents like this are rare in India.
Now imagine if somebody is working on the building roof (work at height jobs) or trestles. Normally in coastal areas, wind speeds are very high and may reach even 200 kmph. This, if coupled with low pressure and accompanying heavy winds can lead to above incidents. In maintenance jobs of roofs, safety advisor will not allow work on roof during winds and rain because of the apprehension that asbestos/metallic sheet roofs can fly off, persons at the top will not have strong foothold, asbestos sheets can become soft due to rain and lead to fall of persons. But maintenance department views such advise as hindrance to work and pass sarcastic comments. The video link is to be seen by such people so that they can be careful in future.
