Safety permits for safe execution of hazardous and non routine works are statutory requirements. Generally, what is to be done including scope of work, job hazard analysis, skills and experience of persons to be engaged, isolations, emptying and or cleaning requirements, suitable PPE, medical fitness, supervision, verification before start of work, pep talks and job specific training, mock trials if required, etc are mentioned in the permits.
But, this is not being done. There will not be visible unsafe events, leading us to believe our methods are correct. As and when accidents occur, it appears to us that these are one of the rare cases.
It is my opinion that accountability with appropriate penalties only will make everyone to be alert, else, issues are overlooked to complete the works within the timelines.
