Death to persons after going into water well

One person entered for cleaning in one case, ,  and to retrieve a fallen hammer in another case, didn't come out. Others entered one after another,  7 in first case, 3 in second case. All died.

It is common to swim in normal water wells, but, poisonous gases will be present  in these types of wells also.

We should presume that any space having some enclosure shall be treated as dangerous and accordingly precautions required for confined space entry shall be taken including buddy system.

Safety permits for works

Safety permits for safe execution of hazardous and non routine works are statutory requirements. Generally, what is to be done including scope of work, job hazard analysis,  skills and experience of persons to be engaged,  isolations, emptying and or cleaning requirements,  suitable PPE, medical fitness,  supervision, verification before start of work, pep talks and job specific training, mock trials if required, etc are mentioned in the permits.

But, this is not being done. There will not be visible unsafe events, leading us to believe our methods are correct. As and when accidents occur, it appears to us that these are one of the rare cases.

It is my opinion that accountability with appropriate penalties only will make everyone to be alert, else, issues are overlooked to complete the works within the timelines.

Long distance driving-sleep problems

Sometimes,  I drive solo, long distances for 8-10 hours. I get drowsy sometimes,  and stop driving.

However,  I  observed no drowsiness,  if I don't eat on the day of driving.  And vice versa.

Taking water also helped to stop vehicle after two to three hours of driving and relieve the pressure.  This also made me to have rest for few minutes and resume driving.

Another observation is to have windows opened instead of using air conditioner,  and fresh air is also relaxing me.

Casual at work - so what?

Today,  I got a simple statement, but a fact in reality.

The statement was made by our safety committee Chairman and it reads,

If you are CASUAL at work, you will go to CASUALTY.
