Hazid and cause-consequence analysis

It is routine for hazard identification in any indistry maintaining minimum standards. Production person/safety person carry daily inspection to observe for unsafe acts / unsafe conditions. Action for addressing the observation(s) is supposed to be initiated immediately. Till such time of rectification, either alternate arrangements will be in place like administrative controls or work suspension. 

But in practice, nothing will happen. It is rare practice, to stop the work as no work means production  loss. Administrative controls are generally in the form of verbal instructions. It is highly doubtful whether implementation will be monitored. It may not be in the knowledge of top management. 

Hence, it becomes a farce and violations will continue.

Improvements can be expected only when all violations are assessed in terms of financial terms for possible injuries, equipment damage,  environment loss, penalties by regulators, loss of trained manpower, etc and reported to management as atleast as an information item on daily basis.

Permissions for works to address safety issues shall be on fast track with financial powers depending on the authority in the organization structure. This allows to take up jobs without delay so that safety issues can be addressed quickly and have safe production. 

Importance of redundancy-learnings from home experience

Once, several years ago, when I was shaving my beard in our house at native place with a twin blade razor, halfway through, it became blunt. I don't have another razor to complete the task.

I can't gobout with oneside shave. Those days, home delivery apps were not there. 

First time faced this problem. Though, we were taught redundancy and diversity in our training for nuclear facilities, and though, I  had good experience working in safety field, I didn't apply these principles for the case of shaving,  problems that may arise, as narrated above.

Fortunately, my brother was at home at that time. He got a new razor from nearby shop and completed unfinished task.

Subsequently, I started keeping more than two sets of razors in my travel bag.

This is the lesson learnt, i.e. having redundancy. And to be followed in all activities, home or work.

Another experience is w.r.t. start time to reach airport, as below. 

Normally,  I keep sufficient margin to be at airport and start early from home. My house is 40 km from airport and approach is through traffic areas. It takes about 45 minutes in early hours and 75-90 minutes during daytime. For security check in airport, it requires 10-30 minutes, as was my experience, both from crowd and security personnel scrutiny of baggage. Another 5-10 minutes to reach boarding gate depending on its location for the flight. 

Once, my boarding time was 0815 hr. My taxi is supposed to pickup me at 0530 hr. The driver didn't come even at 0545 hr. When I tried to call, he didn't pickup initially snd later he informed that he will reach my house in five minutes. But, he didn't come.  It was raining at that time. Later, I came to know that he slept and didn't start in time. When I called at 0545 hr, though he was at his home, he bluffed saying he is nearer to my house giving false hope.

I booked a taxi online, time to pickup showed 0610 hr but reached only at 0620 hr, and we started. Due to rains, roads are flooded, making it slow to wade through. Then, on a narrow flyover enroute,we got stuck with traffic jam from a state transport bus. Finally reached airport at 0815 hr, completed security check in five minutes and reached boarding gate by 0830 hr by running. Boarding for the plane just started, with some delay. I was saved by fast security check, boarding gate position nearby and delayed boarding from airlines. 

Later,  I gave complaint on our taxi driver to our department transport section and he was removed from contract.

Many think and plan their travel as if they go without problems. Possible delays from traffic jam, vehicle breakdown, and others are not considered by them.

It is my opinion that, it is better to wait at destination rather than sitting at home.
